What Is a Disability Placard?
A disability placard (also called handicapped placard or disabled badge) is a removable tag that hangs from a vehicle’s rearview mirror or rests on the dashboard. It grants parking privileges such as access to designated accessible parking spaces, extended parking durations, or metered parking exemptions, depending on local rules


Arizona State Evaluation Process

Submit Application: Use the official Arizona Disability – Deaf/Hard of Hearing Plate/Placard Application (Form 96-0104), which must include the medical certification section completed by an authorized professional—formerly limited to physicians, but now expanded to include PTs

Meet Disability Criteria: The certifying PT or physician must confirm that the person qualifies under defined mobility-limiting conditions—such as needing an assistive device or having a serious health condition that impairs mobility

Approval by ADOT/DPS: The Arizona Department of Transportation (via Motor Vehicle Division) reviews and issues the disabled placard or plate. Permanent placards have no expiration, while temporary ones are valid for up to six months
