Brian Forte PT, DPT
Brian Forte graduated from the University of Pittsburgh’s Doctor of Physical Therapy Program in 2013 and soon thereafter completed The Manual Therapy Institute’s Manual Therapy Certificate Program. He has over a decade of experience in orthopedics and industrial health and wellness. Brian also holds a Master of Business Administration from the University of Southern Indiana, Certification in Dry Needling from the Spinal Manipulation Institute, and Certification as a Clinical Instructor with the American Physical Therapy Association. He is a Brazilian Jiu Jitsu practitioner with special interests in treating combat athletes, post-operative rehabilitation, and preventative musculoskeletal care as it relates to longevity.

Rachel Forte PT, DPT, OCS, FAAOMPT
Rachel Forte graduated from the University of Pittsburgh’s Doctor of Physical Therapy program in 2014. She is an Assistant Clinical Professor at Northern Arizona University as well as a clinician with a special interest in orthopedics, sports, occupational health and perinatal care. She is a Fellow of the American Academy of Orthopedic Manual Physical Therapists, Board-Certified Clinical Specialist in Orthopedic Physical Therapy, Certified Obstetric Physical Therapist and Certified in Dry Needling. Aside from patient care, she is a FAAOMPT mentor and serves on the scholarship committee for the Arizona Physical Therapy Association. In her free time she enjoys trail running, family time and traveling.

Zaira Macias PT, DPT
Dr. Macias is a graduate of Northern Arizona University’s Doctor of Physical Therapy program. She specializes in sports and orthopedic physical therapy, with a strong emphasis on evidence-based care and individualized treatment approaches. Dr. Macias has completed over 300 hours of advanced manual therapy continuing education and is certified in dry needling, allowing her to effectively address pain, movement dysfunction, and performance limitations. She is a bilingual Spanish-speaking clinician, enabling her to provide high-quality, patient-centered care to a diverse population. Dr. Macias is passionate about helping patients return to the activities they love through hands-on care, progressive strengthening, and movement-focused rehabilitation.

Elisabeth Chapman PT, DPT
Elisabeth Chapman, PT, DPT, is a compassionate physical therapist with advanced training in pelvic floor rehabilitation and comprehensive experience treating patients with all types of conditions. She provides evidence-based, individualized care for orthopedic, neurologic, and pelvic health conditions, with a special interest in helping women navigate pelvic organ prolapse, incontinence, pelvic pain, and postpartum recovery. Elisabeth’s clinical background includes rotations at Mayo Clinic’s Pelvic Floor Department, the Northern Arizona VA Health Care System, and outpatient orthopedics, where she delivered one-on-one care for patients ranging from pediatrics to geriatrics. Her diverse experience—including previous roles as a PT technician, personal trainer, and group fitness instructor—allows her to blend functional movement training with targeted rehabilitation strategies. She is known for her supportive communication style, thorough evaluations, and commitment to empowering patients through education and active participation in their care

Anna Fern-Buneo PT, DPT
Dr. Fern-Buneo is a Board-Certified Pediatric Clinical Specialist with over 30 years of clinical experience. She holds a Doctor of Physical Therapy degree from Northeastern University, a Master’s degree in Motor Learning from Columbia University–Teachers College, and a Bachelor of Science in Physical Therapy from Hunter College (CUNY).
She is an Assistant Clinical Professor and Director of Clinical Education in the Physical Therapy program at Northern Arizona University–Phoenix Bioscience Core Campus, where she teaches pediatrics. Anna’s clinical experience spans the full continuum of pediatric care, including roles at Phoenix Children’s, Children’s Hospital Los Angeles, Children’s Healthcare–Minneapolis, and Arizona Early Intervention.
Anna’s clinical interests include developmental delay, high-risk infants, and children with complex medical needs. She serves as co-chair of the Arizona Physical Therapy Association Pediatric Special Interest Group and was named Arizona PT of the Year in 2025.
